8D Reports

The 8D methodology uses a structured eight-step approach to problem solving. The goal is to confront the problem and discover weaknesses in the management systems that allowed the problem to arise. The output of the 8D process is an 8D report.

The steps in the 8D Report are also called “disciplines” so they are called the 8D Report and their steps are as follows.

2D: Problem Description

3D: Temporary Restriction Actions

4D: Root Cause Analysis

5D: Corrective Actions

6D: Verification of Corrective Actions

7D: Preventive Actions

8D: Team and individual recognition
